About the Service
Registration of Births and Deaths is mandatory under the **Registration of Births & Deaths Act, 1969**. Nagar Panchayat Mairwa facilitates the issuance of these certificates for events occurring within its municipal jurisdiction.
Important: Events must be reported within **21 days** of occurrence for free registration. Late registration attracts a penalty and requires additional approvals.

ABirth Certificate
- Discharge Summary from Hospital
- Identity Proof of Parents (Aadhaar)
- Marriage Certificate (if applicable)
BDeath Certificate
- Cause of Death Certificate (Form 4/4A)
- Cremation/Burial Receipt
- Aadhaar Card of Deceased
